What a treatment plan should include
At a minimum: the patient's details and the date, the findings or diagnosis for each tooth, the proposed procedure, the estimated cost, the planned order, and the status of each item. Add the alternatives you discussed and any notes about risks, so the patient can make an informed choice.
Tips for presenting the plan
Number the steps in a sensible order, mark which items are urgent, and give a total with a clear note about what is an estimate. Go through the plan with the patient and keep a signed copy. Check which consent and record-keeping rules apply where you practice.
Before you hand over the plan
- The patient's name and the date are filled in
- Every tooth uses one numbering system, for example FDI
- Procedures are listed in the planned order
- Costs are marked as estimates where they may change
- Alternatives and risks are noted
- The patient and the dentist have both signed
